Application Forms
To apply for a child care license, you'll need to complete the license application form for your specific program type (center, group home, or family home). These forms are available from the DESE Office of Childhood website or from your licensing specialist.
Staff Record Forms
You must maintain records for every staff member, including:
• Employment application with references
• Background check documentation (both FCSR and fingerprint results)
• Training records and certificates
• Health assessment/physical exam records
• First aid and CPR certification
These records must be kept on-site and available for inspector review.
Parent/Guardian Forms
For each child in your care, you'll need:
• Enrollment/registration form with emergency contacts
• Authorization for pick-up (who can and cannot pick up the child)
• Medical information and allergy documentation
• Immunization records
• Parent handbook acknowledgment
Ongoing Record-Keeping
Day-to-day forms you need to maintain include:
• Daily attendance records (sign-in/sign-out)
• Medication authorization forms (signed by parent for each medication)
• Incident/injury reports
• Fire drill logs (drills must be conducted regularly)
• Emergency contact lists
• Menu/meal records (if you serve food)
Keep these organized and easily accessible — inspectors will ask to see them.
